WHO CAN BE A HOST HOME PROVIDER?

Thus, we coordinate regular social gatherings for our host home providers, clients and families to provide the opportunity to get to know one another, and establish partnerships, mentorships and friendships. For both SSN and EIN enrollments a massage license through the Colorado Department of Regulatory Agencies is required for each individual performing services. The coordinator ensures that the individual’s needs are being met in the home and that their plan of care is being followed. They will visit the individual and the Host Home Provider in the home regularly. Cheyenne Village Nurses will coordinate healthcare for the individual and the Benefits Coordinator will manage Medicare, Medicaid, and Social Security benefits. While the person newly placed in your home might already participate in these activities, it is likely that they have not yet found a program.

The applicant must be at risk of transition to a nursing facility, hospital, or Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). To utilize waiver benefits, members must be willing to receive services in their homes or communities. Some may be very independent and able to go out into the community on their own and even have a job, while others will need more support. Our staff will match you with a person whose needs fit your lifestyle. Host Home Providers are independent contractors, not employees, with Community Living Alternatives, Inc. . As a Host Home Provider, you are required to provide supervision and training to the person in your home.
